Limp Bizkit star Sam Rivers' death was announced by his fellow bandmates on October 18. More details have emerged as the 911 call has since been released. According to the call, Rivers was found in a bathroom at his home in Florida.
In the audio, obtained by TMZ, the caller said she found him lying facedown in the bathroom. During phone call, when asked if the music star was breathing, she tragically replied "I don't know."
The tearful caller explained they had just arrived at the house and found Rivers "face first in a pile of blood". The dispatcher urged them to take a deep breath before revealing fire and rescue services would be deployed.
Rivers was pronounced dead at the scene and reportedly had a cut on his eye. TMZ also reported that he may have fallen off the toilet.
The guitarist formed Limp Bizkit alongside Fred Durst and John Otto in 1994. Wes Borland and DJ Lethal joined the band two years later.
Rivers left the group in 2015 before rejoining in 2018.
He had previously revealed that his departure from Limp Bizkit in 2015 was the result of liver disease brought on by years of heavy drinking, for which he later received a liver transplant before returning to the band full time from 2018 until his death.
His last performance with the group came at the 2025 Leeds Festival.
The band described him as the "true legend of legends" in a post announcing his death.
"Sam Rivers wasn't just our bass player - he was pure magic," the band said. "His talent was effortless, his presence unforgettable, his heart enormous.
"We shared so many moments - wild ones, quiet ones, beautiful ones - and every one of them meant more because Sam was there."
